Camp Charm Participation Waiver and

Release of Liability

 

Workshop Title: Camp Charm

Date of Workshop: July 28th, 2025

Location: 1604 Greensboro Avenue, Tuscaloosa, Al

 

In consideration of participating in the above-mentioned workshop, I, the parent or guardian of the participant, understand and agree to the following terms:

 

1. Acknowledgement of Risks:

The participant understands that participation in this workshop may involve certain inherent risks and dangers, including but not limited to the use of tools, chemicals, equipment, physical activity, etc. These risks may be caused by the participant's actions or inactions, the actions or inactions of others, or the conditions of the workshop facilities

 

2. Assumption of Risk:

The participant voluntarily assumes all risks, dangers, and hazards associated with participation in this workshop, including any potential loss, injury, illness, or death, regardless of cause.

Participation is at the participant's own risk, and no promises or guarantees have been made regarding health, safety, or risks. 

 

3. Release of Liability:

The parent or guardian of participant, releases and discharges The Charm Academy, its officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, losses, or damages arising out of or related to participation in this workshop, including any personal injury, illness, property damage, or death.

This release includes claims based on the negligence of The Charm Academy or its personnel, but does not extend to liability for intentional misconduct or violation of the law

 

4. Medical Consent:

The participant consents to receive any necessary medical treatment that may be deemed advisable in the event of injury, accident, or illness during this workshop.

 

5. Compliance with Instructions:

The participant agrees to comply with all instructions and directions given by the workshop instructors and personnel. Failure to follow instructions may result in immediate removal from the workshop.

 

6. Photo/Video Release: 

The participant understands that their photograph or likeness may be captured during the workshop. The participant grants The Charm Academy permission to use these images for promotional and marketing purposes.

 

7. Parent/Guardian Consent (for Minors)

If the participant is under 18 years of age, a parent or guardian must sign the enrollment form to demonstrate compliance with these terms.

 

8. Governing Law:

This agreement shall be governed by and construed in accordance with the laws of the State of Alabama. The parent or guardian of the participant has carefully read and fully understands the terms of this Waiver and Release of Liability. The parent or guardian of the participant understands that certain legal rights are being given up by signing the enrollment form. The parent or guardian of the participant has signed or will sign that digital form voluntarily and of their own free will.
